Consider that you are assuming both physical and legal risks which have potential financial implication for yourself and/or your family should you be injured or killed while staying at ADVENTURE HIGH INC. remote cabin.

I acknowledge that I have voluntarily applied to stay at ADVENTURE HIGH INC. remote cabin, and in consideration of being permitted to stay at the remote cabin, do voluntarily execute this "Liability Release and Assumption of Risk" on behalf of myself, my heirs and next of kin, my personal representatives and my estate.

That I hereby acknowledge that certain risks of injury and unforeseeable environmental hazards that are inherent to staying at ADVENTURE HIGH INC. remote cabin. These injuries could be minor or serious and may result from one's own and/or others actions or the inaction's. By acknowledging these specific risks I agree to personally assume such risks.

I also understand that access to the cabin can be delayed for the EMT (emergency medical technicians) due to poor trail conditions.

I also understand that ADVENTURE HIGH INC. will transport me and my gear to the cabin at the start of my stay and again at the end of my stay. All other movement to and from the cabin will be my sole responsibility. This transportation will be in the way of 6-wheel all terrain vehicle or 4-wheel all terrain vehicle. That I hereby acknowledge that I understand that ADVENTURE HIGH INC. does not carry specific liability insurance on the all terrain vehicles. I also understand that I will have to leave my vehicle in the parking area on Bradfords Cove Road.

I also acknowledge that getting to and from the remote cabin can be physically strenuous activity and requires physical fitness and good health.

I declare that I am in good physical and mental health. I believe that I am able without reservation able to stay at ADVENTURE HIGH INC. remote cabin. I agree to inform ADVENTURE HIGH INC. of any medical/mental concerns applicable to my stay at ADVENTURE HIGH INC. remote cabin. I hereby authorize Adventure High Inc. staff to secure such medical advice and services as may be deemed necessary for the health and safety of myself, my child or other in the group and I agree to accept any financial responsibilities. Please list any concerns medical or other wise that could affect your participation in ADVENTURE HIGH INC. activities.

I hereby release ADVENTURE HIGH INC. its officers, employees, servants and agents from any liability with respect to any damage and/or injury (including death) that I may suffer during my stay except where the damage or injury is caused by the gross or wilful negligence of ADVENTURE HIGH INC. its officers, employees, servants and agents acting within the scope of their duties.

I further state that I am of sufficient age and mental capacity to sign this liability release, or that I have acquired consent of my parent.

I also hereby acknowledge that there could be certain costs applied to my bill:
~$0.25per min. plus long distance charges, except for emergency or phone calls to Adventure High office.
~$35 charge if you request that we come out to the cabin, for purposes other than emergencies, and/or problems with the cabin
~$20 per night per person for the third and fourth person. And $45 person for each additional person.
